What to Bring
- A valid photo ID (driver’s license or state ID)
- Your insurance card(s)
- A list of your current medications and dosages
- Any relevant medical records or test results
- Payment for any copay or self-pay services
Before Your Visit
- Arrive about 15 minutes early to complete new-patient paperwork
- Fill out any forms we send you ahead of time
- Write down questions or concerns for your provider
